Critical Thinking Questions-
-
Your client has a nasogastric tube and you are to give a tube feeding.
Before starting the feeding you aspirate secretions from the tube. They
are straw colored and have a pH of 7.4. Is the tube in the stomach? How
do you know?
-
Your client is receiving bolus tube feedings every 4 hours. When you go
in to give him the next feeding, he is complaining of a feeling of fullness.
His bowel sounds are hypoactive. What further assessments would you make
before giving the feeding?
-
You are inserting a nasogastric tube and meet resistance as you insert
it through the left nostril. What would you do?
You have just finished inserting the nasogastric tube and you find that
the client cannot speak. What happened?
-
Your client has just returned from having myelogram. His post procedure
orders include that he maintain bedrest with the head of the bed flat
for 24 hours. He has an IV at 20cc/hour (keep vein open). He also has continuous
tube feedings via a nasogastric tube at 60cc/hour. Would you hold the tube
feedings?
-
Your client has been receiving continuous tube feedings via a Dobbhoff
Feeding tube (small bore). He has started vomiting and the doctor ordered
you to connect the Dobhoff to low intermittent suction. Would you question
the order?
-
Your client has been getting tube feedings via a G tube. You note that
the skin around the G tube is red and irritated. What can cause this and
what would you do about it?
-
Your client has a Salem sump tube connected to low intermittent suction.
You note that there is no drainage in the collection bottle. List all
the possible causes for this and what assessment you would make to determine
if each problem (cause) exists:
Questions I have for my instructor after preparing for lab:

sump hooked up to suction.
-
