10 Points for the Personal Health Habit History (SDLP 2)
Complete the questions and turn
into your lab instructor on the second week.
Mon labs- 9/8

10 points for the interview (Appendix A) Interview one adult
and one child (up to 18
years old). The child should not be your own. Complete
the interview form and questions and turn into
your lab instructor by the third week.

10 points for cultural video. Guidelines for this assignment are
in the lab portion of the
syllabus (SDLP 2). You may do option
one, option two, or option three. Check out the videos and answer
the questions.
Turn into your lab instructor by the last day of
lab.

20 points for lab quiz on last day of lab. Covers oral and topical
medications.

The average of the 4 assignments must be 75% in order to receive a passing grade in lab.
