Academic Probation Self Assessment

We understand sometimes things will happen that impact your performance in class.  When these roadblocks occur, your advisor wants to make sure they can help you get back on track and be successful.  Because your academic performance has fallen below the necessary 2.0 GPA (C average), we are requiring you to complete this self assessment tool.  Please provide honest answers to the questions on this self-assessment. Your answers will assist your academic advisor in identifying problem areas, as well as assist with creating a strategy for academic improvement.  This form must be completed and printed prior to you scheduling an appointment to meet with your academic advisor.
